* lib/rubygems/installer.rb (Gem::Installer#initialize): env may
not be under /usr/bin.
* lib/rubygems/installer.rb (Gem::Installer#shebang): uses /bin/sh
if shebang has any options, since env shebang trick does not
work with options.
