Made utrace probes more robust.

* tapsets.cxx (utrace_derived_probe_group::emit_probe_decl): Added
death event handlers to ensure that for every utrace_attach there
is a corresponding utrace_detach.
(utrace_derived_probe_group::emit_module_decls): Ditto.
