Note that I now also prefer to use utmp.h if it is present - this seems
to be much better under Linux.

bugfix: fixed problem that caused compilation on FreeBSD 9.0 to fail.

... by moving code to stream.c. Thanks to the new design, new cases are
not really needed, resulting in cleaner code.

I also did a cleanup of header file usage as a side-activity.

this caused some problems with the threading model, e.g. zombies. Also,
it was far less optimal than it is now.
