Create only styles for element types

Up until now a new style was created for every start tag in the HTML
code. Now there are only a handful of styles are created, one for each
element type (h1, h2, code, div, ...). Other styles are applied directly
to characters.

Fix gtAction - automatic line spacing application

In case automatic line size was supposed to be used, the original code
only computed line size that should be used and then used that value as
fixed line size for every paragraph. This patch ensures that if
automatic line size is selected, it will be applied as expected, not by
computing fixed line size.
