Subject: Fix tools sync operations and mem hierarchy

Tools were using nested loops that are illegal.
(and enforced in latest tevent with a nice abort())
Fix them by creating appropriate synchronous transaction calls.
Also fix tools_ctx mem hierarchy setup.
