get rid of MK_CMD_ENTRY macro; update doc/README.command

(autoscript, bmp, bsp, fat, mmc, nand, portio, ...)

sort when sorting command name list). Minor cleanup here and there.

  - remove trailing white space, trailing empty lines, C++ comments, etc.
